Registration for Students who already attended High School

*The following documents MUST be provided at the time of your registration appointment.

1. Birth certificate  
2. Immunization record
3. Parent/Guardian photo ID
4. Lease/notarized letter from landlord or property tax bill (if owner)
5. At least two Proof of City of Elizabeth residency:  utility bills (gas, electric, water) telephone/cell phone bill, insurance bill, medical bill, cable bill.

Please note, in addition, the Universal Health Record filled out by your child's pediatrician found under https://www.state.nj.us/health/forms/ch-14.pdf must be brought at your registration appointment.  Additional Proof of City of Elizabeth residency might be required after 30 days.
