      • Welcome to our EPS Registration Page.  Please choose the appropriate grade level above to register.

      Early Childhood PreK Lottery

      • Early Childhood PreK Program Registration for the CURRENT School Year 

        To register your child into our Early Childhood PreK Program, you must come in person to:

        Frances C. Smith Early Childhood Center - 1000 S Elmora Ave, Elizabeth, NJ 07202

        Please note registration is currently open for children born on or before October 1st, 2021

        • Children who's birth dates are between 10/02/2020 - 10/01/2021 will be placed in Prek 3 (no exceptions). 
        • Children who's birth dates are between 10/02/2019 - 10/01/2020 will be placed in Prek 4 (no exceptions).

         

        PRESCHOOL LOTTERY REGISTRATION (for 2025-2026 School Year) 
        Registration appointments are now available for children born between 10/02/2021 and 10/01/2022. These children are eligible for possible placement in Pre-K 3 starting September 2025.  (no exceptions).

        *Registration will be in person and by appointment only, starting from February 19, 2025 through March 21, 2025. You can book your appointment using the link below.

        Registration — Required Documents for all Grades (Pre-K through 12) 

        The child’s birth certificate, his/her immunization records, physical examination (within one year), if new to the United States of America, tuberculosis (TB) testing is required, and proof of residency as listed below.

        Please provide one of the following to meet your five point requirement:

        ___ Deed
        ___ Current Property Tax Bill
        ___ Current Lease or 5-C form

        ___ Mortgage
        ___ Landlord Affidavit (form 6-C)”

        Four of the following (All must have your name and address)

        ___ Government Issued Identification
        ___ Current Gas bill
        ___ Current Cable Bill 
        ___ Current Water/sewer Bill
        ___ Homeowner Insurance
        ___ Paystub from Employer

        ___ Credit card bill
        ___ Current Electrical Bill
        ___ Benefit Statement
        ___ Current Cell Phone Bill
        ___ Auto Insurance
        ___ Bank Statement

        Please provide a completed Universal Health Record form filled out by your child's pediatrician at your registration appointment.  The Universal Health Record form can be found under state.nj.us/health/forms/ch-14.pdf

        A parent/ guardian must be present for the registration process.  No exception will be made.

      K-8 Registration

      • To register in grades from Kindergarten to 8th Grade, we invite you to visit us in person:

        Grades Kindergarten (DOB On or prior to 10/01/2019) through 8th Grade

        Parent guardians must bring in all the required documentation
