    PRESCHOOL IN PERSON LOTTERY REGISTRATION 

    *The following documents MUST be provided at the time of your registration appointment.

    • Birth certificate  
    • Immunization record
    • Universal Health Record filled out by your child's pediatrician found under https://www.state.nj.us/health/forms/ch-14.pdf
    • Parent/Guardian photo ID
    • Lease/notarized letter from landlord or property tax bill (if owner)
    • 2 Proofs of City of Elizabeth residency:  utility bills (gas, electric, water) telephone/cell phone bill, insurance bill, medical bill, cable bill

  • *Registration will be in person, by appointment only, beginning February 22, 2023 through March 24, 2023.  Appointments will be available through the link below.
