You must submit the following transcript request form to your child’s previously attended school

Please allow 10 days between submission of the transcripts request form and scheduling a registration appointment with the Elizabeth Public Schools.  Your child’s previously attended school will need to send the Elizabeth Public School Registration office official transcript records.  Please email welcomecenter@epsnj.org or call 908-436-5278 after 10 days of submitting the transcripts request form to your child's previously attended school to verify that the Elizabeth Public School has received the transcript paperwork from your child's previously attended school.
