Registration for students Kindergarten to 8th Grade

*The following documents MUST be provided at the time of your registration appointment.

1. Birth certificate  
2. Immunization record, physical examination (within one year), if new to the United States of America, tuberculosis (TB) testing is required, Physical Examination (within one year) and proof of residency as listed below. 
3. Parent/Guardian photo ID
4. Lease/notarized letter from landlord or property tax bill (if owner)
5. At least two Proof of City of Elizabeth residency:  utility bills (gas, electric, water) telephone/cell phone bill, insurance bill, medical bill, cable bill.
6. Previous School Transcripts/Report Cards/ Test Scores if a school in USA

Please note, in addition, the Universal Health Record filled out by your child's pediatrician found under https://www.state.nj.us/health/forms/ch-14.pdf must be brought in at your registration appointment.  Additional Proof of City of Elizabeth residency might be required after 30 days.
