Dear Parent/Guardian,
Please be advised that the New Jersey Department of Health requires all children born on or after January 1, 1997, and enrolled in the sixth grade to receive a booster dose of tetanus, diphtheria, and pertussis vaccine (Tdap), as well as one dose of meningococcal (Menactra) vaccine for school attendance.
You have the option of going to your private physician or to a clinic where immunizations are given free of charge. The schedule for these clinics and the necessary forms can be obtained from the nurse’s office.
The City of Elizabeth Department of Health and Human Services offers a free immunization clinic, by appointment only, to eligible Elizabeth residents aged two months to 18 years old. Residents must have Medicaid/Family Care (Part A only) or be uninsured to qualify for free vaccines. If your child had previous immunizations, the records should be brought to the clinic appointment. You may make an appointment by visiting elizabethnjvaccine.org.
Please contact the school nurse if you have any questions regarding these required immunizations. If your child has already received the above immunizations, please send in a written report from your health care provider so we can update our records.
Your child must be in compliance with vaccine requirements as noted above or they will be excluded from school on the first day in September 2023.
