Early Childhood In Person Registration Appointment

PRESCHOOL IN PERSON LOTTERY REGISTRATION 

Lottery is open to children whose birthdates are between 10/02/2021 and 10/01/2022, for possible placement in Pre-K 3 starting September 2025 for the 2025-2026 school year (no exceptions).

*The following documents MUST be provided at the time of your registration appointment.

• Birth certificate  
• Immunization record
• Universal Health Record filled out by your child's pediatrician found under https://www.state.nj.us/health/forms/ch-14.pdf
• Parent/Guardian photo ID
• Lease/notarized letter from landlord or property tax bill (if owner)
• 2 Proofs of City of Elizabeth residency:  utility bills (gas, electric, water) telephone/cell phone bill, insurance bill, medical bill, cable bill
